Must be 18 years of age or permission from an adult if qualified. Participation in a sport or yoga may result in accident or injury. I assume the risk is connected with participation in a sport or yoga and represents that I am in good health and suffer from NO physical impairment that would limit my use of any facility or activity practiced or used by Yogafitness4Me, LLC.

I specifically agree that YOGAFITNESS4ME, its officers, employees, and agents shall not be liable for any claim, demand, or cause of action of any kind resulting from or related to participant use of the facilities or participation in any sport, exercise, or activity within or without the club premises, and participant agrees to hold YOGAFITNESS4ME harmless from same.

I have read the above release and waiver of liability and fully understand its contents.
